OHS Incident Reporting - New arrangements
From 3 August 2009 the process for reporting OHS incidents will change. An enhancement to the Human Resources and Student Administration System now allows direct reporting of incidents via HORUS (and ISIS for students) rather than the current Apollo System. Users will notice little change to the current arrangements other than logging into HORUS/ISIS rather than Apollo and a few minor changes to the information requested. Current links to ‘OHS Incidents’ on the ANU staff homepage and the OHS webpage will continue to operate and will direct users to a HORUS and ISIS login screen. Alternatively users can login to HORUS/ISIS directly and select the ‘Submit Incident Notification’ option from the user menu.
A user guide will be available which should help with any initial queries. Health, Safety and Work Environment (HSWE) Branch can also be contacted for advice on ext 52193. This new functionality will make the system more efficient and improve turnaround times in incident processing.
Please note that the University’s Incident Reporting Procedure requires every death, injury, exposure or dangerous occurrence to be reported within the following timeframes:
- in the case of a fatality, immediately after emergency action has been taken. A telephone call to the HSWE Branch on ext 52193 must also follow immediately;
- in the case of any injury, exposure or dangerous occurrence, reported within four hours.
